Our 4 Part Straight Bills of Lading - Short Form meets IDOT and ICC regulations for land, sea and air shipments. It is used to document shipments of non-hazardous materials. It provides space to list all essential shipping detail and provide a special column to denote hazardous materials. It is available in a 8 1/2" x 7 5/8" or 8 1/2" x 11 5/8" version.

It is sold in packages of 50 forms.
